SIDR2660 - CASE OFFICER

The Case Officer provides family centered support services to women, children and families to ensure effective transition of services. The incumbent is responsible for initiating and coordinating assessments, responding to patient, family and health care member’s enquiries, providing anticipatory guidance and case management, across the health care continuum. /he/ She works in collaboration with other members of the health care team, both within and outside of Sidra and acts as a primary liaison between agencies.

The Case Officer strives to provide evidence-based care by integrating research with practice. Promotes continuous improvement and helps to generate innovative ideas within case management practice and patient and family centered care practices. Maintains a commitment to life-long learning and creates, utilizes and/or disseminates new knowledge to patients and their families, the public and other professionals internal /external health professionals.  The Case Officer is responsible for ensuring that all duties are carried out to the highest possible standard.

The Case Officer participates in the provision of teaching and supervision of peers on wide range of subjects within the clinical field.

KEY ROLE ACCOUNTABILITIES:
Initiates and coordinates assessments, responds to patient, family and health care member’s enquiries, providing anticipatory guidance and case management, and liaises with other support diagnostic/health services in coordinating patient referrals
Collaborates with other members of the health care team to evaluate family-centered patient management in the particular setting by performing activities such as reviewing patient/family history, developing individualized service plans for specific patients/families, and meeting with other members of the health care team to discuss and review the recommendations
Coordinates teaching of patients/families with a new complex diagnosis including the development of teaching resources, referring to and liaising with other health care professionals, acting as a resource for other health care professionals, identifying learning needs and conducting teaching sessions
Provides supportive case management to families by ensuring they receive assistance with patient advocacy, information regarding program, team, or community health and educational resources, and referrals to appropriate community services and/or agencies
Acts as a liaison, as appropriate, between the program, the interdisciplinary team, and community health and educational services and agencies to clarify information, responsibilities and recommendations to ensure the needs of the patient/family are addressed
